Policies Designed with You in Mind

Insurance

Your insurance policy is a contract between you and your insurance company.  We are not a party to that contract.  In the event we do accept assignment of benefits and your insurance company has not paid your account in full within 30 days, the balance will be transferred to your account.  Please be aware that some, and perhaps all, of the services provided may be non-covered services under the terms of your insurance policy.  Our practice is committed to provide the best treatment for our patients and we charge what is usual and customary for our area.  You are responsible for payment regardless of any insurance company arbitrary determination of usual and customary rates. 

PAYMENT

Cash, check, and care credit are also accepted forms of payment. We understand that temporary financial problems may affect timely payment of your balance.  We strongly encourage you to communicate any such problems to us so that we can assist you in the management of your account. 

CAnCELLATION

To provide the best quality service to all our patients, we require at least 24-48 hours’ notice for cancellations or for rescheduling your appointments. 

Treatment Estimates

We will be happy to file an estimation of benefits on your behalf, but it is only an estimate. The treatment estimates are given to you based upon the best and only information available to us from your insurance company. When dealing with insurance companies your estimate may differ substantially from what you get from them due to any number of reasons, for which we do not have any information.  Some common ones are Exclusions and Limitations of your insurance policy-waiting periods, insurance company denials of medically necessities, treatment code restrictions, previous treatment already billed to your insurance company, uses of alternatives fee schedules by your insurance company, age restrictions, etc.

MINORS

The Parent/Legal Guardian that accompanies the minor to the appointment is responsible for any payment due on day of service.  For unaccompanied minors, non-emergency treatment will not be provided unless treatment and applicable charges have been pre-authorized by the Parent/Legal Guardian before the appointment date and payment arrangements have been made with the Business Office Associate.
